To qualify for benefits, you must:

• Meet eligibility requirements
  
• Be a Hawaii resident
  
• Be at least 19 years of age
  
• Be a U.S. citizen or qualified alien

Children's Benefits:

• Eligible until age 21 for routine dental care
  
• Includes checkup every six months

Adult Benefits:

• For adults 21 years and older, benefits are limited to emergency treatment to reduce pain from infection or injury.

Who to Call:

• For help in finding a dentist who accepts your insurance coverage, call CCMC at 486-8030 (Oahu) or 866-486-8030 (neighbor islands).
• If you haven't moved and haven't received your insurance card, call DHS, (808) 524-3370.
• If you have changes in your address, phone number or primary insurance, call your eligibility worker.
• Transportation can be provided for ONLY emergent dental situations when no other means of public or private/personal transportation is available.
• If you need an interpreter, call CCMC.
